Physical Therapist - PRN
Crittenden Community Hospital has an immediate opening for PRN Physical Therapist.

JOB SUMMARY:

Organizes and conducts medically prescribed physical therapy programs to restore function and prevent disability following disease or injury, helps patient reach his maximum performance and learn to function within the limits of his/her capabilities. Understands and provides age appropriate care to all ages including prenatal, neonatal, pediatric, adolescent, adult and geriatric patients.

ESSENTIAL JOB FUNCTIONS:

1.Meets clients goals and provides quality care by assessing and interpreting evaluations and test results, determining physical therapy treatment plans in consultation with physicians and by prescription.

2.Administers physical therapy treatments according to a treatment plan approved by the referring physician.

3.Supervises physical therapist assistants according to State Practice Act regulations for supervision.

4.Evaluates effects of physical therapy treatments by observing, noting, and assessing clients progress; providing medically necessary treatment plan modifications.

5.Completes discharge planning by consulting with physicians, nurses, assistants, social workers and other healthcare workers; contributing to client care conferences.

6.Documents client care services by charting in client and department records according to accepted regulatory, corporate and professional guidelines. Records daily treatments per department procedures.


7.Maintains client confidence and protects facility operations by keeping information confidential.

8.Updates case managers on patient progress for continued coverage.

9.Conducts or participates in training medical and nursing personnel in physical therapy techniques and objectives.

10.Adheres to JCAHO standards.

11.Promotes and participates in performance improvement activities.

12.Supports the corporate mission: to provide quality, cost-effective healthcare in a positive and productive work environment.

14.Must attend all mandatory meetings.

15.Complies with all applicable state and federal laws and conducts job activities in an ethical manner as outlined in Crittenden Community Hospital policies and procedures and in accordance with the standards and practices stipulated in the Corp. Compliance Plan.

ADDITIONAL RESPONSIBILITIES:

1.Performs other related duties as assigned.

2.Home Health work is performed in the home care setting in the counties of Caldwell, Crittenden, Livingston, and Lyon.

3.Must have working automobile available for use on daily basis, responsible for maintenance on vehicle. Responsible for providing proof of insurance coverage upon request.

4.Must have a valid Driver’s License

QUALIFICATIONS:

EDUCATION, TRAINING OR EXPERIENCE:

1.Graduate of an Accredited Physical Therapy program.

2.Licensed with the State of Kentucky to practice physical therapy.

3.Completes corporate general orientation and also department specific orientation including job competency.

PHYSICAL DEMANDS:

Should be able to lift, sit, stand, stoop, and walk without restriction.
