Crittenden Community Hospital has an immediate opening for OT position.
Performs and develops effective client treatment plans to restore, maintain or prevent decline of client function, by planning and administering medically prescribed occupational therapy services for all patients in the following age groups: neonatal, pediatric, adolescent, adult and geriatric in accordance with current federal, state and local standard governing the facility.
ESSENTIAL JOB FUNCTIONS
1.Performs Occupational Therapy Evaluations.
2.Establishes occupational therapy treatment plans based on the evaluation and in conjunction with physicians and by prescription.
3.Supervises assistants according to Kentucky State Occupational Therapy Practice Act.
4.Evaluates effects of occupational therapy treatments by observing, noting and assessing client’s progress.
5.Provide medically necessary treatment plan modifications.
6.Documents client care services by charting in client and department records according to accepted regulatory, corporate and professional guidelines.
7.Records daily treatment per corporate procedure.
8.Maintains confidence and protects facility operations by keeping information confidential.
9.Complete discharge planning by consulting with physicians, nurses, assistants, social workers, and other health care workers; contributing to client care conferences.
10.Updates case manager on patient progress for continued coverage.
11.Conducts or participates in training medical and nursing personnel in occupational therapy techniques and objectives.
12.Adheres to JCAHO standards.
13.Promotes and participates in performance improvement activities.

16.Must attend all mandatory meetings.
17.Complies with all applicable state and federal laws and conducts job activities in an ethical manner as outlined in Crittenden Community Hospital policies and procedures and in accordance with the standards and practices stipulated in the Corp. Compliance Plan.
1.Performs other related duties as assigned.
2.Home Health work is performed in the home care setting in the counties of Caldwell, Crittenden, Livingston, and Lyon.
3.Must have working automobile available for use on daily basis, responsible for maintenance on vehicle. Responsible for providing proof of insurance coverage upon request.
4.Must have a valid Driver’s License

EDUCATION, TRAINING OR EXPERIENCE
1.Graduate of an Accredited Occupational Therapy Program.
2.Licensed with the State of Kentucky to practice Occupational Therapy.
3.Completes corporate general orientation and also department specific orientation including job competency.
Should be able to lift, sit, stand, stoop and walk without restriction.
