Case Management Supervisor
Crittenden Community Hospital has an immediate opening for Full-time, Case Management Supervisor position. Competitive salary and benefits package available.

JOB SUMMARY:
The Supervisor of Case Management is responsible for the leadership and staff management of Case Managers which includes a MDS Coordinator. Supervision also includes monitoring staff utilization of services, case mix index and length of stay.
Also, performs duties of social service and discharge planning.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
1.Reviews patient records for medical necessity and makes decisions regarding the level of care, length of stay and appropriateness of care. Effectively communicates information to the physicians and patients/families, to ensure quality of care of the patient.
a.May include making rounds with a physician on a daily basis.
2.Conducts and participates in Interdisciplinary Care Planning meetings and Care Management Department meetings to share information gathered and to assist in the coordination of the care of the patients.
3.Coordinates and provides supervision over staff.
4.Assesses and begins discharge planning/social services on daily basis and to include referrals for end of life care, social services and mental health consults.
5.Maintains required regulatory requirements in documentation and confidentiality at all times.
6.Keeps the spiritual, cultural and psycho social needs of the patient and family as top priority.
7.Make referrals to other facilities, agencies and community resources as indicated.
8.Consults, if needed, after hours to Nursing Staff for any social service or discharge planning issues.
9.Performs required procedures that meet regulatory mandates to facilitate reimbursement for utilization of services by the customers of Crittenden Community Hospital. Includes insurance reviews by telephone, written documentation, faxing or via computer use.
10.Find appropriate coding of Diagnostic Related Groups (DRG), diagnoses, assigns temporary DRG and discusses with Medical Records coder if discrepancy.
11.Performs monthly medical record reviews to collect, aggregate, analyze and display data to assist in performance measurement activities, utilization practices and patterns.
12.Attend all mandatory in-services, managerial meetings.
13.Participate on appointed committees.
14.Prepare and evaluate department budget and staff utilization.
15.Promote and participate in performance improvement activities.
16.Prepare Medicare Minimum Data Set on swing bed patients.

EDUCATION, TRAINING OR EXPERIENCE:
1.Kentucky RN licensure required.
2.Previous Case Management experience preferred.
3.Prior supervisory experience preferred.
4.Computer skills to fulfill job duties.
5.Activity Certification

Physical Demands:
1.Lifting, carrying, pushing or pulling objects may be required.
2.Work is primarily sedentary (involves sitting most of the time, but may involve walking or standing for brief periods of time).

Work Environment:
Work is typically performed in an adequately lighted and climate controlled office.
